Appointment Information
The Brooke Surgery’s appointment system is open 24 hours a day. You can create a request for yourself, or someone you care for, at any time.
Our team will get back to you Monday to Friday, between 8am to 6.30pm.
When patients use our online service, this in turn keeps the phone line free for the vulnerable patients who do not have internet access.

Bringing a Chaperone to your Doctor’s Appointment
We always respect individuals’ needs and requirements, and making sure our patients feel comfortable is our top priority. If you feel you would like to have a chaperone present during your doctor’s appointment, please let us know when you make your request.
Sickness Certificates (Fit Notes)
You must give your employer a doctor's 'fit note' (sometimes called a 'sick note') if you've been ill for more than 7 days in a row and have taken sick leave. This includes non-working days, such as weekends and bank holidays.

Home Visits
Whilst we encourage our patients to come to the surgery, where we have the proper equipment and facilities available, we do appreciate this is not always possible. This is why we have a certain number of house calls that can be made from a doctor or an appropriate clinician. If you do need a home GP visit, you can assist us by calling reception before 10:30am, and we can discuss arranging a home doctor or an appropriate clinician to come and see you. All requests for a home visit are triaged by a GP. The decision as to whether a home visit is warranted rests solely with the doctor or an appropriate clinician on call and is based on clinical assessment.
Who is eligible for a home visit by the doctor?
You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP or an appropriate clinician will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor or an appropriate clinician to schedule house calls.
When will a home doctor visit?
The doctors or an appropriate clinician usually make home visits between morning and afternoon surgeries. We cannot guarantee which doctor or an appropriate clinician will visit you.
You can also be visited at home by a community nurse if you are referred by your GP. You should be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.
Please remember that demand for GP appointments is very high. A GP can see up to 4 patients in the surgery in the time it takes to make one home visit, and may be unable to do a full examination in your home without the specialist equipment that is available at the surgery.

Ordering a Repeat Prescription
The easiest ways to order repeat prescriptions are:

- If your medication is not on repeat and available to order via the NHS please use our online services AskmyGP

Dental Issues
Unfortunately we are not able to manage dental issues here, as we do not have the right training, indemnity nor appropriate equipment. You would have to contact your dentist.
If you are not registered with a dentist, you can contact Greater Manchester's Urgent Dental Care Service on 0333 332 3800 to get help. They are open from 8am to 10pm every day, including weekends and bank holidays.
They operate from 14 different locations across Greater Manchester, so you can choose to be seen at a location that’s convenient for you.
They should be able to give you self-help advice and treatment (where appropriate) within 24 hours. Hope they are able to help
Eye Problems
There is a Minor Eye Conditions Service where you can be seen by a specialist on the same day for eye problems, like you described. They may be better placed to help than we can, as they will be better able to examine your eyes. And can prescribe treatment.

These are walk in clinics, first come first serve, so please go early to get seen.
Please note you may have to wait up to 2 hours.
Change or Cancel an Appointment
If you are not able to attend your nurse or doctor’s appointment, please let us know through our online service. Or speak to our reception team on 0161 368 3312.
Out of Hours
Life Threatening
Call 999 or go to A&E now if:
- you or someone you know needs immediate help
- you have seriously harmed yourself - for example, by taking a drug overdose
A mental health emergency should be taken as seriously as a medical emergency.
Evening & Weekend GP Appointments
You can book appointments in advance for weekday evenings (6:30pm to 8pm) at these locations:
Thornley Medical Centre, Glossop, and Ashton Primary Care Centre.
If you need an appointment on Saturday, call 0161 368 5426 between 9am to 4pm. Appointments are at the Healthy Hyde Body & Mind Centre (Unit 42-44 Clarendon Mall, SK14 2TA).
Non-urgent
Use NHS 111 if:
- You need help now, but it's not an emergency
There will be someone to provide you with advice and to direct you to a clinician if it is necessary.
