New Patient Registration

If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photographic ID.

You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. The forms are available on the above "Registering Online" tab or you can download them from here:

New Patient Pack

All new patients are required to book a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

If you wish to register for online services, please complete the form in the new patient pack and then call back for your log in details a week later or contact reception. On line access is via 

Please note, if you do not attend your new patient appointment we will assume that you have decided not to register with us and your application will be destroyed.

Medical treatment is available from the date of registration. Please contact reception for further information.

If you move

Please let us know your new address. If you move outside the practice area, we will advise you how to register with a new doctor.
